Monte Cook explores the differences between player success and character success in this blog post. In my opinion, this isn't talked about enough. I think the more important angle is that just because a character fails, doesn't mean the player fails. Splitting these two outcomes apart from each other allows for more dynamic roleplay and interfacing with the game itself.
