Monte Cook explores the differences between player success and character success in this blog post. In my opinion, this isn't talked about enough. I think the more important angle is that just because a character fails, doesn't mean the player fails. Splitting these two outcomes apart from each other allows for more dynamic roleplay and interfacing with the game itself.

I realized I've never actually posted on the website about this video I released a while back but I still stand by what I talk about and, who knows, maybe some more eyes on the video will get me to finally make Part 2 and Part 3...Your conflict in your campaign is arguably the most important part. This video will help you make a conflict that your players truly connect with.
